Work alongside a dedicated and energetic team of Reablement Assistants and Occupational Therapists, providing therapy-led services to those in need and collaborating with a broader multidisciplinary team, including local hospital discharge teams. Together, youll create therapy pathways that maximise individuals independence.
A valid licence to drive in the UK is required as the role involves travelling locally and lone working.

Your Responsibilities Will Include:
- Supporting people with their personal care and empowering them to do as much as possible on their own.
- Monitoring and providing feedback on the progress of peoples goals.
- Assisting with medication and encouraging independence.
- Completing care records and other relevant paperwork.
- Facilitating access to community groups and services.
What Were Looking For:
- Qualifications: Degree in Occupational Therapy.
- Registration: HCPC registered Occupational Therapist.
- Skills: Strong interpersonal, communication, and ICT skills.
- Knowledge: Health and Social Care legislation, safeguarding adults.
- Requirements: Valid UK driving licence and use of own vehicle.
